    HELLO!

    Good morning doc.

    I’m sending a Sickness Notification Form from the SSS to be certified and signed by you thru my brother Mr. Dennis G. Alquizola, SSS- Iligan is advicing to file for an extension of leave and now I’m asking for an extension of leave since I was examined last February 12, 2009 that my calcium level is below and so with the TSH and FT4 is not balance as per laboratory test result last February 12, 2009.

      To Carol… kindly call office. TSH is suppressed because you have the disease that requires the TSH to be low. The calcium level I checked was normal. I think the person in SSS mistakenly thought a low TSH is abnormal for your case when in fact thats what we want.
